High School Boys, Girls Basketball & Wrestling

Wednesday, November 12th is the first official practice day.

 

The following paperwork is required in the office before the student may begin practice on November 12th.

1.  Current sports physical signed by physician, student and parent

2.  Code of conduct signed by student and parent

3.  District permission form signed by student and parent

4.  Emergency contact form (a new one is needed for each sport)

5.  $150 check payable to Lyons Middle Senior. 

Approved free/reduced lunch students have the following fee:   $75 for reduced and $37.50 for free lunch applications.  

If your student participated in a fall sport, please verify that they have a current sports physical for their winter sport and complete number 4 and 5.
